Description

Work Setting: Inpatient / Acute Care

Schedule:

4x10 hour shifts (weekly rotating schedule)
Day Shift: 07:00 – 17:00 (start between 7 AM – 9 AM)
Night Shift: 20:00 – 06:00
Rotates 1 week days / 1 week nights
Weekends required
Final schedule discussed during interview

Requirements:

RPh License (Texas) – Required
PharmD – Preferred
Experience in inpatient / acute care pharmacy
Meditech Expanse experience preferred

Clinical Responsibilities:

Anticoagulation therapy dosing
Cardiac dosing
Pharmacokinetic dosing
Renal dosing
Therapeutic medication monitoring
Warfarin dosing
High-alert medication management

IV & Medication Preparation:

IV antibiotics preparation
Chemotherapy preparation
Medication pre-packing

Medication Systems:

Experience with Pyxis dispensing systems

Pharmacotherapy Management:

Renal disease medication management
