Job Summary
We are seeking a qualified Speech-Language Pathologist (SLP) to provide therapy services in a Skilled Nursing Facility (SNF) setting. The SLP will evaluate, diagnose, and treat patients with speech, language, communication, and swallowing disorders. This role requires strong clinical skills, collaboration with interdisciplinary teams, and familiarity with electronic documentation systems.
Key Responsibilities
- Evaluate patients to determine the presence and extent of speech, language, cognitive-communication, and swallowing disorders.
- Develop and implement individualized treatment plans to improve communication and swallowing abilities.
- Provide therapeutic interventions to patients recovering from neurological conditions, stroke, injury, or illness.
- Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.
- Maintain accurate and timely clinical documentation and progress notes in the facility’s electronic health record system.
- Collaborate with physicians, nurses, occupational therapists, physical therapists, and other healthcare professionals to coordinate patient care.
- Educate patients, families, and caregivers on communication strategies, swallowing precautions, and therapy exercises.
- Participate in care planning meetings and interdisciplinary team discussions.

Minimum Requirements
- Active Speech-Language Pathologist license in the state of California
- Minimum of 1 year of experience in a Skilled Nursing Facility (SNF)
- Experience using Rehab Optima (Net Health) documentation system preferred
- Strong communication, assessment, and patient care skills
- Ability to work collaboratively within an interdisciplinary rehabilitation team
