Responsibilities

Assess patients’ physical, psychosocial, spiritual, and educational needs using holistic nursing assessment.
Recognize and report abnormal findings or changes in patient condition to physicians and charge nurses.
Collaborate with healthcare team members using appropriate handoff and communication methods.
Identify safety risks related to the patient, family, and environment and implement safety initiatives.
Develop and prioritize individualized patient care plans based on collected assessment data.
Coordinate and revise multidisciplinary plans of care in collaboration with patients and families.
Set measurable patient care goals and communicate plans using SBAR and handoff processes.
Initiate discharge planning upon admission and coordinate referrals when necessary.
Provide evidence-based nursing care according to hospital policies and established standards.
Implement physician orders and delegate nursing tasks appropriately using critical thinking.
Educate patients and families based on individual care plans and health needs.
Respond effectively to emergency situations with timely and precise nursing interventions.
Implement infection control measures and maintain a safe patient care environment.
Continuously evaluate patient outcomes and adjust care plans according to evidence-based standards.

Requirements

Experience
Ability to provide age-specific and culturally competent patient care
Strong communication and collaboration skills with healthcare teams
Ability to maintain patient confidentiality
Ability to adapt to change and work in diverse healthcare environments

Licenses/Certifications
Active RN License – Ohio or Compact License
BLS (American Heart Association)

Education
Diploma in Nursing – Required
BSN – Preferred
