Job Summary

• Local travelers are NOT accepted – Candidates living within 50 miles of the facility cannot apply.

🩺 Patient Care Responsibilities

• Create and manage individualized patient care plans.
• Adjust the care plan based on how the patient responds to treatment.
• Evaluate effectiveness of treatment and outcomes.

👩‍⚕️ Care Coordination

• Coordinate and supervise patient care when required.
• Work with the healthcare team to ensure proper patient treatment.

📋 Organization & Prioritization

• Ability to organize workload and prioritize tasks efficiently during shifts.

📚 Assessment & Education

• Use patient assessment data to determine care needs.
• Provide patient education and discharge planning.

⚠️ Monitoring Patient Condition

• Identify abnormal findings during assessments.
• Report issues and update the care plan accordingly.

📝 Documentation

• Accurately document all patient care activities, including:
  • Wound assessments
  • Patient education
  • Progress toward care goals
  • Medication administration
  • Nursing flow sheets
  • Assessment forms
