Description
LPN needed for a Skilled Nursing Facility in Quakertown, PA. You will provide direct nursing care for long term care and rehab residents, including medication administration, treatments, wound care support, monitoring for changes in condition, and clear communication with the RN and provider team. This is a busy SNF environment where strong time management, teamwork, and accurate documentation are essential. Candidates should be comfortable managing med passes, supporting admissions and discharges, and delivering compassionate care to a primarily geriatric population.
Active LPN license required (PA)
BLS preferred (facility dependent)
Recent SNF or LTC experience preferred
Comfortable with med passes, injections, tube feeds, wound care, vitals, and basic assessments within LPN scope
EMR charting and accurate documentation required
Floating within the facility may be required based on census
Must be able to pass background, drug screen, and complete all facility compliance items on time
Flexibility with scheduling, including weekends and holidays as needed
