Travel Physical Therapist (13-Week Contract) – West Lafayette, IN

Position Overview

We are seeking a skilled and motivated Physical Therapist for a 13-week travel assignment in West Lafayette, Indiana. This role is responsible for providing comprehensive evaluative and rehabilitative services to patients under physician direction. The patient population includes individuals undergoing both non-operative care and post-operative recovery, particularly related to spine and extremity conditions.

Key Responsibilities

• Perform initial evaluations and develop individualized treatment plans based on physician protocols
• Implement pre-operative and post-operative rehabilitation programs
• Deliver therapeutic interventions focused on improving mobility, strength, and function
• Accurately document all treatments, patient progress, and status updates in the EMR system
• Monitor patient progress and identify changes in condition, reporting findings to physicians and nursing staff
• Collaborate with interdisciplinary teams to ensure continuity and quality of care
• Educate patients on their condition, recovery process, and rehabilitation goals
• Communicate effectively with family members regarding patient progress and independence in daily living activities
• Participate in departmental quality assurance and internal audits
• Maintain a clean, organized, and safe therapy environment
• Manage and coordinate daily patient scheduling

Qualifications

• Bachelor’s degree in a science-related field required; Master’s degree in Physical Therapy preferred
• Active Physical Therapist license in the State of Indiana required
• 1–2 years of clinical Physical Therapy experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and within a team-oriented environment

Assignment Details

• 13-week travel contract
• Opportunity for extension based on performance and facility needs
• Full-time schedule

Schedule

• Monday: 7:00 AM – 4:00 PM
• Tuesday: 9:00 AM – 6:00 PM
• Wednesday: 7:00 AM – 4:00 PM
• Thursday: 9:00 AM – 6:00 PM
• Friday: 7:00 AM – 4:00 PM
• 8-hour shifts, Monday–Friday

Assignment Details

• 13-week travel contract
• Weekly Pay: $2,339
