Simplifi Clinical Interviews - Manager to conduct final interview
Unit: Home Health Occupational Therapy
Experience Required: 1 year home health
Certs Required: BLS, NC State License
Skills Required: HCHB (HomeCare HomeBase experience required (no exceptions)
ADL training, AIDL training, endurance training, assessing need for and providing education on adaptive equipment, create plan of care to get patients to highest level of functioning in the most efficient way, good communication with clinical team, recognizing signs and symptoms to prevent readmissions.
Patient Types: Chronic diseases, Orthopedics/Ortho Surgery including total hips/knees, falls, weaknesses, CHF, COPD, occasionally stroke patients

Focura - will upload discharge instructions and other important docs such as wound pictures
