Position Overview

Eligibility Requirements

• Perm candidates from any OTHER Providence facility (NON-SWEDISH) are eligible if separated for at least 3 months
• Prior hospital experience required - specifically Inpatient/Acute Care and Case Management

Required Certifications

• BLS
• LSWAICMSW or LCSW

Technical Requirements

• Epic Experience: Required
• Patient Ratio Experience: 20:1

Scheduling Details

Weekend Coverage

• Required: Minimum 2 weekend days per month
• Typically scheduled separately (1 Saturday per month + 1 Sunday per month, not in the same weekend)
• Patient Ratio Weekends: 50:1

Floating Requirements

Travelers may be floated to other Swedish campuses if census is low:

• Issaquah
• First Hill
• Ballard

• Refers to rehab and long term care facilities
• Refers to other disciplines/resources

Documentation

• Documents pertinent information to facilitate care coordination
• Documentation according to plan of care
