We are looking for a licensed Physical Therapist to join our compassionate and growing team in the Green Bay area. This is a great opportunity to build meaningful connections with patients while enjoying autonomy and flexibility in your schedule. As a clinician with PRN HH&T, you’ll primarily work in a home health setting, with additional opportunities for cross-training and work in skilled nursing facilities if desired.

You Are a Physical Therapist Who:
- Has great written and verbal communication
- Enjoys building relationships and trust with patients and families
- Works well independently while also collaborating with a supportive team
- Brings compassion, reliability, and clinical skill to every patient interaction
Job Responsibilities:
- Perform in-home patient evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to improve patient mobility, function, and quality of life
- Collaborate with patients, families, and clinical team members to support care goals
- Accurately document treatments and progress, including discharge summaries
- Supervise and guide Physical Therapist Assistants (PTAs) in treatment plans
- Educate patients and caregivers on safe mobility practices and discharge planning
Requirements:
- Current Physical Therapist (PT) license in the state of Wisconsin
- Valid driver’s license and reliable transportation
- 1-3 years of Home Health experience preferred but not required—we provide training!
