Description

Job title: Home Health/Hospice RN

Provide patient care and case management using the nursing process and individualized Plan of Care for assigned home health, hospice, and palliative care patients.
Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with department policies and procedures.
Conduct initial evaluation visits and regularly re-evaluate patients’ nursing needs.
Initiate and update the plan of care as necessary.
Furnish services requiring substantial and specialized nursing skills.
Implement appropriate preventive and rehabilitative nursing procedures.
Prepare clinical and progress notes.
Coordinate services and communicate patient condition and needs to physicians and other personnel.
Counsel patients and families to help meet nursing and related needs.
Participate in in-service programs and continuing education.
Supervise, mentor, and teach other nursing personnel.
