Telemetry RN – Unit Notes

Location: Boise, Idaho
Unit: Telemetry – Floors 3 & 7 (pooled staffing)

Unit Overview

  • 3rd Floor: 20 beds
  • 7th Floor: 35 beds
  • Staffing: Floors 3 & 7 share staff pool
  • Travelers are expected to float between both floors

Patient Ratios

  • RN : Patient = 1:4

Required Certifications

  • BLS
  • ACLS
  • NIHSS

Patient Population

3rd Floor – Cardiac Focus

  • Post-procedural cardiac patients
  • Post-op Day 1 open heart
  • TAVR
  • Ablations
  • Post-cath lab patients
  • Pacemakers
  • Medical telemetry patients

7th Floor – General Telemetry

  • Post procedural overflow
  • CHF
  • Sepsis
  • Electrolyte imbalances
  • Post stroke patients
  • Other general telemetry diagnoses

Required Experience / Skills

Must have experience with:

  • Post procedural cardiac patients
    • Cath lab
    • Ablations
    • Open heart
  • Telemetry interpretation
  • Documenting rhythm strips

Preferred:

  • PCU / Stepdown experience

Drips

RN titration required?

  • Heparin
  • Insulin (rare – facility can train)

Common drips

  • Amiodarone
  • Cardizem
  • Dopamine
  • Dobutamine
  • Set rate cardiac gtts

Support Staff

Charge Nurse

  • Yes – on each unit
  • Usually no patient assignment
  • Crisis & resource nurses available

CNAs

  • 3rd Floor: 3 CNAs
  • 7th Floor: 5 CNAs
  • Typical ratio: 1:8 (varies by census)

Respiratory Support

RT manages respiratory equipment.

Equipment available:

  • High flow oxygen
  • BiPAP
  • CPAP

RN role:

  • Mostly troubleshooting
  • May assist RT if needed
  • RT administers breathing treatments

Telemetry Monitoring

  • Remote tele techs located on 3rd floor
  • RNs must read and interpret telemetry
  • 2 rhythm strip interpretations per shift

Schedule

  • Shift: 7 AM – 7 PM / 7 PM – 7 AM
  • Schedule length: 6 weeks
  • Travelers help fill schedule gaps
  • Block scheduling not guaranteed
  • Maximum 5 shifts in a row

Scheduler tries to cluster night shifts together

Weekend Requirement

  • 5–6 weekend shifts per 6-week schedule

Weekend definition:

  • Days: Saturday & Sunday
  • Nights: Friday & Saturday

Holiday Requirement

  • May work holidays
  • Not expected to work every holiday

Floating

Travelers are first to float

Possible float units:

  • Med-Surg
  • Med-Surg Tele
  • Telemetry units

Possible facilities:

  • Boise
  • Meridian
  • Nampa

Orientation

  • 2 shifts total
    • 1 shift on 3rd floor
    • 1 shift on 7th floor

  • Any color allowed
