Description

LPN needed for a Long Term Care setting in Claremont, NH. You will provide direct resident care including medication administration, treatments, wound care support, monitoring for changes in condition, and clear communication with the RN and provider team. Expect a steady pace with busy med passes, strong teamwork, and consistent documentation. The ideal candidate is dependable, organized, and compassionate with a focus on resident safety, comfort, and dignity.

Active LPN license required (NH) BLS preferred (facility dependent) Recent LTC or SNF experience preferred Comfortable with med passes, injections, tube feeds, wound care, vitals, and basic assessments within LPN scope EMR charting and accurate documentation required Floating within the facility may be required based on census Must be able to pass background, drug screen, and complete all facility compliance items on time Flexibility with scheduling, including weekends and holidays as needed
