Licensed Practical Nurse (LPN) – Long Term Care (Contract, Varied Shifts)

Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) for a contract assignment in a Long Term Care (LTC) setting. The LPN will provide comprehensive nursing care to residents, supporting daily clinical needs and ensuring high-quality, patient-centered care.

Key Responsibilities

Perform patient assessments and monitor changes in condition
Administer medications and treatments in accordance with physician orders
Communicate and report pertinent patient information to physicians and nurse practitioners
Assist with admissions and discharges
Support residents with activities of daily living (ADLs)
Provide specialized care including:
- Gastrostomy tube (G-tube) care
- Urinary catheter care
- Nebulizer treatments
- Wound vacuum (wound vac) management
- CPAP therapy support
Maintain accurate and timely documentation using electronic medical records
Perform additional duties as assigned based on patient and facility needs

Schedule

Varied shifts based on census and facility needs
Holiday coverage may be required

Minimum Qualifications

Active LPN license (state-specific requirements apply upon acceptance)
Minimum of 2 years of Long Term Care (LTC) experience
Experience working in a skilled nursing or long-term care environment
Familiarity with Meditech EMR preferred

Certifications Required

BLS (Basic Life Support)

Additional Requirements

Proof of influenza vaccination or an approved medical/religious exemption required prior to assignment
Ability to attend pre-employment health screening and onboarding requirements as scheduled
