Description

Job Title: Social Worker – Licensed Clinical Social Worker (LCSW)

Location: 1200 Northside Forsyth Drive, Cumming, GA 30041
Duration: 13 Weeks (Contract)
Schedule: Evening Shift – 5×8 Hours (12:00 PM – 8:30 PM)
Hours: 40 Hours per Week
Department: 748F BHS – Hospital Based

Important Note: Local candidates within 50 miles of the facility will not be considered.

Job Summary

We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join our hospital-based Behavioral Health Services team. The Social Worker will provide psychosocial assessments, crisis intervention, care coordination, and discharge planning for patients and families. The role involves collaborating with multidisciplinary healthcare teams to ensure patients receive appropriate emotional, social, and community support throughout their care.

Required Qualifications

  • Active LCSW (Licensed Clinical Social Worker) license in the State of Georgia
  • BLS (Basic Life Support) Certification
  • Minimum 2 years of experience within the specialty
  • Hospital-based social work experience required
  • Strong clinical assessment and crisis intervention skills
  • Ability to work in a fast-paced healthcare environment

Roles and Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and environmental needs.
  • Provide crisis intervention and counseling to patients and families experiencing acute stress or mental health concerns.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary teams to develop patient-centered care plans.
  • Facilitate discharge planning including coordination with community resources, rehabilitation centers, and support services.
  • Assist patients and families in understanding treatment options, care plans, and available support systems.
  • Advocate for patients to ensure access to appropriate mental health and social services.
  • Maintain accurate and timely documentation in electronic medical records (EMR) in accordance with hospital policies.
  • Support patients dealing with behavioral health conditions, trauma, grief, and chronic illness.
  • Provide guidance on community programs, housing resources, financial assistance, and support groups.
  • Ensure compliance with hospital policies, state regulations, and professional ethical standards.
