Job Opportunity: LPN/LVN (Licensed Practical Nurse) – Discharge Unit (RDU)

Location: Klamath Falls, Oregon
Contract Length: 13 weeks


Job Summary

The Licensed Practical Nurse (LPN) in the Medically Ready for Discharge Unit (RDU) works under RN supervision to provide direct patient care for patients medically ready for discharge but experiencing delays due to various barriers. The role involves collaboration with a multidisciplinary team and assigning/supervising tasks to LPNs, CNAs, and unregulated assistive personnel (UAPs).


Required Qualifications

  • Valid LPN/LVN license in Oregon
  • BLS Certification
  • Minimum of 1 year of Long Term Care experience, with experience within the last year
  • Medical-Surgical (Med Surg) experience

Responsibilities

  • Provide direct patient care under RN’s plan of care, including focused assessments and implementation of prioritized interventions
  • Develop and evaluate focused plans of care as directed by RN
  • Assign and supervise tasks to LPNs, CNAs, and UAPs according to licensure and job descriptions
  • Accurately document assessments, interventions, patient responses, and progress in the electronic health record
  • Assist with patient and family education related to discharge plans, medication instructions, and follow-up care
  • Communicate effectively with interdisciplinary team members to ensure continuity of care and smooth transitions
  • Collaborate with RNs, case managers, social workers, and other team members for discharge planning
  • Adhere to hospital policies, procedures, and protocols, including quality improvement and safety initiatives
  • Report concerns promptly to appropriate staff

Physical and Mental Demands

  • Good general health required
  • Ability to frequently stoop, bend, reach, stand, walk, and sit
  • Ability to lift at least 50 pounds and push/pull hospital equipment
  • Ability to function during periods of stress, trauma, and/or disease

Schedule

  • Nights every other week (EOW) required
