Job Summary

We are seeking a motivated and skilled Certified Occupational Therapy Assistant (COTA) to join a dynamic skilled nursing team. The role involves providing patient-centered occupational therapy services, collaborating with the care team, and maintaining accurate documentation in compliance with facility standards.

Requirements

Active California COTA License
Minimum 1 year of experience in a Skilled Nursing Facility (SNF) setting
Knowledge of Rehab Optima (NetHealth) or similar EMR systems
Strong organizational, documentation, and communication skills
Ability to work collaboratively with interdisciplinary teams
Day shift availability, including holiday coverage

Responsibilities

Provide occupational therapy interventions to SNF patients based on individualized treatment plans
Collaborate with the interdisciplinary care team to optimize patient outcomes
Maintain accurate and timely documentation in the facility EMR system
Adhere to facility policies, procedures, and therapy protocols
