Job Summary

We are seeking a motivated and skilled Certified Occupational Therapy Assistant (COTA) to join a dynamic skilled nursing team. The role involves providing patient-centered occupational therapy services, collaborating with the care team, and maintaining accurate documentation in compliance with facility standards.

Requirements

  • Active California COTA License
  • Minimum 1 year of experience in a Skilled Nursing Facility (SNF) setting
  • Knowledge of Rehab Optima (NetHealth) or similar EMR systems
  • Strong organizational, documentation, and communication skills
  • Ability to work collaboratively with interdisciplinary teams
  • Day shift availability, including holiday coverage

Responsibilities

  • Provide occupational therapy interventions to SNF patients based on individualized treatment plans
  • Collaborate with the interdisciplinary care team to optimize patient outcomes
  • Maintain accurate and timely documentation in the facility EMR system
  • Adhere to facility policies, procedures, and therapy protocols
