RN Case Manager – Direct Hire | Outpatient & Acute Care

Job Summary:

We are seeking a dedicated Registered Nurse (RN) Case Manager to join a dynamic healthcare team. This role is responsible for coordinating patient care across acute care, swing bed, and outpatient settings, ensuring high-quality outcomes, efficient resource utilization, and compliance with regulatory standards.

The ideal candidate will have strong experience in case management, discharge planning, utilization review, and care coordination.

Key Responsibilities:

• Manage patient care across the continuum, including acute care, swing bed, and outpatient services
• Perform discharge planning to ensure safe and timely transitions of care
• Conduct utilization review to support appropriate level of care and cost efficiency
• Collaborate with interdisciplinary teams including physicians, nurses, and social workers
• Ensure compliance with CMS guidelines and regulatory standards
• Coordinate patient services, referrals, and follow-up care
• Advocate for patients to ensure optimal healthcare outcomes
• Maintain accurate and timely documentation using EMR systems

Required Qualifications:

• Active Registered Nurse (RN) license (or willingness to obtain)
• BLS (Basic Life Support) certification required
• Minimum 2 years of clinical or case management experience
• Strong knowledge of care coordination, discharge planning, and utilization review

Preferred Qualifications:

• CCM (Certified Case Manager) or ACM (Accredited Case Manager)
• Experience working in acute care or hospital-based case management
• Familiarity with Cerner or other EMR systems
