Job Summary:

• Monitors patients for and provides education on food/drug interactions. Records food and drug interactions in the education section. Consults with physicians regarding appropriate diet modification

Minimum Requirements:

• 1 years of experience, 2 years preferred.
• Outpatient experience required
• Experience with dialysis patients required
• Bachelor's Degree in Nutrition or related field required
• Preferred Education: Master's Degree.
• Required Certifications & Licensure: Dietitian/Nutritionist NYSED.
• Registration by the Commissions on Dietetic Registration (CDR) as a Registered Dietitian

• BLS through AHA

   

Job Responsibilities:

• Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients' nutritional needs and status and provides follow-up care. -Interviews patients and/or their families to obtain history of food habits and preferences. -Interprets pertinent laboratory, physical and social data in the medical record, as it applies to nutrition care. -Provides nutrition education and records patient's diet and diet instructions and the patient's understanding of the diet in the education section. -Modifies patient menus as needed.
