Description

Job Summary:

We are seeking a compassionate and dedicated Social Worker to provide direct social work services to pediatric and adolescent students and their families. In this role, you will help address social, emotional, and environmental challenges that impact students’ well-being and academic success.

The Social Worker will collaborate with students, families, and school staff to develop and implement effective care plans, provide community resource referrals, and support students through the social work process.


Key Responsibilities:

  • Provide direct social work services to pediatric and adolescent students
  • Assess student and family needs and develop appropriate care plans
  • Collaborate with school staff and interdisciplinary teams to support student outcomes
  • Coordinate care and ensure effective use of available resources
  • Provide referrals to community agencies and support services
  • Support students and families through social, emotional, and lifestyle challenges
  • Maintain effective communication with students, families, and school personnel

Required Qualifications:

Education:

  • Master’s Degree in Social Work (MSW) from an accredited program

Licensure:

  • Active Michigan Social Worker License:
    • LMSW (Licensed Master Social Worker) OR
    • LLMSW (Limited Licensed Master Social Worker)

Additional Requirements:

  • Valid Michigan Driver’s License
  • Reliable vehicle with insurance as required by the State of Michigan
  • Knowledge of pediatric and adolescent care

Preferred Qualifications:

  • Previous home care experience
  • Experience working with school-aged children or adolescents
  • Strong communication and collaboration skills

Ideal Candidate:

  • Passionate about supporting students and families
  • Strong understanding of social work processes and care coordination
  • Ability to work collaboratively with schools and community partners
