Assignment Details

Schedule: 4×10s | 09:00 – 19:30
Days vary, schedule posted 4 weeks in advance
Weekend rotation: 1 weekend every 4 weeks
Contract length: 13 weeks
Territory: Seattle / Shoreline area

Requirements

Previous Hospice RN experience required
Previous Admissions RN experience required
HCHB (Homecare Homebase) required
Active RN license (WA or compact if applicable)
Strong assessment and documentation skills

Job Duties

Complete hospice admission visits in patient homes / facilities
Perform comprehensive assessments
Review hospice eligibility criteria
Coordinate start-of-care with interdisciplinary team
Educate patients and families on hospice services
Complete all documentation in HCHB
Communicate with physicians, case managers, and hospice team
