Candidates must have the following:

1. Proper State License
2. Skilled Nursing/long term care experience – since October 2019 (PDPM experience)
3. Rehab Optima (Net Health)
4. Negative COVID 19 test required (usually within 7-10 days of start)
5. COVID-19 Vaccination card/documentation - medical and religious exemption accepted
