Job Summary:

We are seeking a dedicated Home Health Physical Therapist (PT) to provide high-quality, patient-centered care in a home setting. The ideal candidate will evaluate, treat, and manage patients independently while ensuring optimal recovery and outcomes.

Key Responsibilities:

• Evaluate and treat patients in a home health setting
• Develop and implement individualized care plans
• Monitor patient progress and adjust treatment as needed
• Complete accurate and timely documentation using OASIS
• Educate patients and families on treatment plans and exercises
• Travel between patient homes within assigned territory
• Collaborate with interdisciplinary healthcare teams

Requirements:

• Active California PT License (Preferred at submission; pending license accepted if already applied)
• OASIS experience required (may consider strong Home Health candidates without OASIS)
• Home Health experience preferred
• If no Home Health experience, Outpatient experience required
• Strong clinical and documentation skills

Compensation & Work Expectations:

• Guaranteed Weekly Hours (GWW): Based on productivity
  • 25 completed visits = 40 hours guaranteed
• Productivity-based schedule
• Mileage reimbursement between patient visits (IRS Standard Rate)
• Rates may vary based on territory coverage and experience

Additional Notes:

• COVID exemptions permitted
• Must be comfortable working independently in the field
• Flexibility required based on patient scheduling and territory needs
