Job Details

• Specialty: Registered Nurse (RN) in Medical / Surgical / Telemetry
• Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Must provide front and back images of the card.
• NIHSS (National Institutes of Health Stroke Scale): Optional but preferred.
• TNCC (Trauma Nursing Core Course): Optional.
• License/Certificate Verification: Must be dated within 30 days of start. State verification required (e.g., NURSYS for RNs).

Required Health & Immunization Documentation

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, methadone. No THC allowed.
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption (declination not accepted in CA or IL).
• MMR: Proof of 2 vaccinations or IgG titer lab results.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption (declination not accepted in CA or IL). Valid for flu season 10/1/2024 - 4/30/2025.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB screening with chest x-ray if positive (within last year for certain states, within 5 years for others).
• Varicella: Proof of 1 vaccination or IgG titer lab results.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

• Joint Commission CORE Exam: Within last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.
