Description
Pride Health is hiring a Home Health Physical Therapist to support our client’s medical facility, located in Puyallup, WA 98373. This is a 3+ month contract assignment (possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Job title: Home Health Physical Therapist
Location: Puyallup, WA 98373
Duration: Contract
Shift: Mon - Fri 8am - 4:30pm.
Guaranteed Hours -36
Travel: Patient homes - Pierce County, WA
Mileage Reimbursement: Yes (between patient visits).
Specific Duties:
- Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines.
- Develop and implement individualized treatment plans in collaboration with physicians.
- Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.
- Monitor and document patient progress, reporting changes to physicians and nurse supervisors.
- Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.
- Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries.
- Travel to patient homes within the assigned service area and nearby locations as needed.
- Participate in care coordination, team meetings, and case conferences.
- Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as applicable.
- Maintain compliance with agency policies, state regulations, and professional standards of practice.
- Participate in continuing education to maintain licensure and clinical competency.
Qualifications:
- Active Physical Therapist license in Washington State (or compact license).
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- Experience in home health or related clinical settings preferred.
