Shift Breakdown: 5x8 hrs
Shift: 8 hours, days

8 hour and 10 hour shifts available, no holidays, weekend rotation required. Later shift would be heavily supporting the ED. 8's: 8am-4:30pm or 3pm-11:30pm (ED) 10's: 3pm-1:30am (ED) 5+ years case management experience with 2+ years recent acute inpatient case management experience, Master of Social Work, LMSW licensure in the state of MI, Discharge planning experience. MANAGER IS VERY STRICT ON THE RECENT INPATIENT CASE MANAGEMENT EXPERIENCE - TELEPHONIC, UTILIZATION REVIEW, PSYCHOLOGICAL EXPERIENCE DOES NOT APPLY TO THIS REQUIREMENT.
