JOB DESCRIPTION
RN - Continuing Care

● About Facility:
· Skilled Nursing Facility
· 625 Bethany Rd, Albemarle, NC 28001
· Radius rule: Traveler's permanent address must be 50 miles outside of home unit
● Department Information:
· Beds: 90
· Minimum Years of Experience required? 18 months in SNF/LTC Rehab facility
· Will you accept a first-time traveler? Not preferred
● Patient Types/Common Diagnoses:
· Short-term and Long-term services offered to patients recovering from an illness, injury or accident
· Stroke, Amputation, Neurological Disorders, s/p Joint Replacement, etc
· No Memory Care
● Patient Ratios:
· RN: 1:18-20 Days; 1:26 Nights
· LPN: 1:18-20 Days; 1:26 Nights
· CNA: 1:13 Days; 1:15 Nights
● Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements
· RN: BLS
· LPN: BLS
· CNA: BLS
● Skills required:
· Previous Long-Term Care exp to include: Trach management (refresher provided during orientation), foley care, wound experience, dressing changes, IV starts/management, tube feedings
● Support within the Department:
· Nursing Leadership
· Admin Nurse will do any admission – admissions will be approx. 1-2 per day
· HCT
· Therapies
· Case Management
● Technology/Equipment:
· EMR: EPIC
· IV Pump: Alaris
· Med dispensing: Omnicell
● Floating:
· Within Continuing Care and scope of practice
· Will be required to float per region needs – will require floating to all facilities listed below, as needed, within scope of practice – DECLINE if not agreeable
· Rehab Region - Cleveland Pines, Huntersville Oaks, Jesse Helms, Sardis Oaks, and Stanly Manor
· If floating mid-shift, Traveler will be paid for the time traveling between facilities (will remain on the clock)
● Orientation (facility and unit):
· 2 days departmental
● Other notes:
· Scrub Color: LPN - Ceil Blue
· Parking: Free
