In this vital role, you’ll be instrumental in managing admissions, discharges, and providing top-tier care to our residents, including hospice and rehab-to-home services. With a flexible schedule of 4 or 5 nights a week—every other weekend—you’ll enjoy a balanced work-life experience while contributing to a supportive and collaborative environment.

Required Skills:

• Valid Illinois RN license
• Minimum of 1 year of nursing experience in LTC/SNF, hospice, or rehab settings
• Strong assessment and patient care skills
• Excellent communication and teamwork abilities
• Ability to handle admissions and discharges efficiently

Nice to Have Skills:

• Experience with electronic medical records (EMR) systems
• Knowledge of hospice and rehab-to-home protocols
• Prior experience working in a fast-paced nursing home environment

Preferred Education and Experience:

• Active Illinois RN license
• 1+ years in LTC/SNF or similar healthcare settings

Other Requirements:

• Must be eligible for temp-to-perm employment
• Willingness to work night shift hours
• Comfortable working in a facility that may require floating and adherence to facility policies
