Description

Case Manager LCSW - Days - 40H, no weekends
INPT CM and EPIC experience
3YR experience required; will accept first-time traveler with inpatient hospital/acute care experience; EPIC charting preferred
Average Daily Census – 15 cases daily
Patient Diagnosis - INPT case management providing and coordinating medical home care, placement & social issues-wrap around type services for safe discharges.
Scrubs/Attire: Business casual

Holidays: required as they fall, as scheduled
Floating: required
Weekends: not required, but opportunity for shifts if interested, On-call: not required
