Job Summary

Seeking a Physical Therapist (PT) for a travel assignment in Cheraw, South Carolina. The role involves evaluating and treating patients, developing individualized physical therapy plans, supervising treatment teams, and maintaining professional standards in patient care and documentation.

Contract Length

Key Responsibilities

• Maintain a professional image and excellent customer relations with patients, visitors, physicians, and co-workers.
• Collaborate with staff to develop professional skills, new programs, and patient treatment plans.
• Perform treatments safely and timely, involving staff and family members as appropriate.
• Document medical records in accordance with APTA standards of care.
• Participate in meetings, educational sessions, and departmental scheduling adjustments.
• Ensure a clean, safe, and orderly work environment.
• Evaluate patients using specialized testing procedures and data from medical records.
• Develop individualized physical therapy plans of care targeting short- and long-term goals.
• Supervise treatment teams and coordinate with Physical Therapist Assistants (PTAs).
• Provide evidence-based care supported by current research.
• Maintain patient confidentiality and adhere to all policies and procedures.

Required Qualifications

• Valid Physical Therapist license for South Carolina.
• Basic Life Support (BLS) certification.
• Proof of required immunizations including MMR, Hepatitis B, Varicella, and Influenza (seasonal).
• Negative 2-step PPD or IGRA (Quantiferon/Tspot) tuberculosis test.
• Valid driver’s license or state ID.
• Social Security Card.
• Completion of a 10-panel drug screen within 30 days of start.
• Criminal background check covering the last 7 years.
• Employment verification for the past 5 years.
• Skills checklist completed prior to start and updated annually.
• Confirmation of assignment form.

Experience

• Minimum 5 years of experience in Physical Therapy.
