Description

Position: RN-Care Manager (Home Health)

Facility: Adventist Health Clear Lake

Location: 15630 18th Ave, Clearlake, CA 95422

Contact Duration: 13 weeks with possible extension.

Shift Timing: (5x8) (M-F) (8:00 am – 4:30 pm)

Hours per week: 40 hours.

Job Description:
Provides high-quality patient care by supporting the development, implementation, evaluation, and ongoing improvement of the case management program. Serves as a key liaison between patients, families, nurses, physicians, the multidisciplinary team, and payers to ensure coordinated care and optimal patient outcomes. Also acts as a resource and consultant to the healthcare team on patient-specific needs.

Position Details:

Shift: Day shift, 40 hours per week
Location: Lake County, CA (Home Health setting – not hospital-based)
Travel: Required for patient visits; mileage reimbursed at the government rate

Required Qualifications:

Home Health experience is mandatory
Proficiency with HCHB (Homecare Homebase) software
Experience with OASIS documentation
Active RN license in the state of practice
BLS certification (BLS / HS-BLS / RQI BLS)

Preferred Qualifications:

Bachelor of Science in Nursing (BSN)
Prior experience in a case management role
Case Management certification

This role is ideal for an experienced Home Health RN with strong case management skills and the ability to coordinate care effectively across multiple stakeholders.
