Home Health Physical Therapist (PT)
Riverside, CA
Full-Time | 40 Hours per Week
Pay Rate: $65-$75 Per Hour 
 

A growing home health provider is seeking a Physical Therapist to join its team in Riverside, California. This is a full-time, 40-hour-per-week opportunity working with an adult patient population in the home health setting. In this role, the PT will provide skilled, physician-directed therapy services designed to help patients restore function, improve mobility, manage pain, and support greater independence in their daily lives. This position offers the opportunity to deliver high-quality, patient-centered care in the comfort of patients’ homes.

About the Opportunity:
This home health opportunity is with an established provider serving adult patients in the community. The team is committed to delivering compassionate, comprehensive care that helps patients improve function, increase safety, and maintain independence at home.

Responsibilities:

• Provide skilled physical therapy services to adult patients in the home health setting
• Evaluate patient needs and develop individualized treatment plans in accordance with physician orders
• Implement therapeutic interventions to improve strength, mobility, balance, safety, and functional independence
• Educate patients and caregivers on exercises, mobility strategies, and home safety recommendations
• Document visits, progress, and outcomes accurately and in a timely manner
• Collaborate with physicians and the interdisciplinary care team to support quality patient outcomes

Qualifications:

• Active California Physical Therapist license
• Home health experience preferred
• Strong documentation and communication skills
• Ability to work independently and manage a community-based caseload
• Current CPR certification and valid driver’s license preferred
