1. Min 1 year experience required. 2. Required skills: Skilled home care experience- Will not accept private duty nursing experience only Must have home health experience with OASIS training and experience. IV skills and wound care General Medical Dx High risk Dx: HF, COPD, Mi, Pneumonia, Stroke Post-op care 3. BLS required 4. Unit Info (share with your traveler): Patients: cardiopulmonary assessment, basic wound care, ABI compression dressing, wound vac dressing, Pleurx /Aseptdrain care, foley catheter change and care, staple/suture removal, use of glucometer, use of pro-time machine, SQ/IM injections, lab draw venipuncture, IV skills, PICC line or Midline care and dressing change, SQ port access/deaccess Skilled Home Care (Medicare, Medicare Advantage and Commercial ins.) Home skilled visits - 6-8 per day 5. Alternating weekend shifts 6. Must have valid drivers license and car to make home visits.
