Description

Physical Therapist (PT) – Home Health | North Shore, MA | $2,688/week

In a Home Health setting, the therapist will provide care directly in patients’ homes, managing treatment plans to improve mobility, function, and overall quality of life across a wide range of diagnoses.

Quick Details:

Location: North Shore, MA (Beverly, Salem, Marblehead, Swampscott)
Setting: Home Health
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Weekend Rotation: 1 every 6 weeks

Responsibilities:

Evaluate and treat patients in their place of residence
Develop and implement individualized care plans
Collaborate with physicians, families, and interdisciplinary teams
Complete timely electronic documentation

Requirements:

Active Massachusetts Physical Therapist license
1+ year Home Health experience required
Current BLS (AHA preferred)
Supervisory professional references
Ability to travel between patient visits
Must complete Drug Screen and Background Screen
