Job Title: Registered Nurse (RN) – Clinical Assessor Location: Nassau County, NY Schedule: Hybrid (2 days remote, 3 days onsite) Pay: $55–$58 per hour Job Overview We are seeking a Registered Nurse (RN) – Clinical Assessor to perform comprehensive patient assessments to determine eligibility for community-based long-term care services. This hybrid role involves a combination of in-person and virtual assessments across the region. The position is focused on evaluations and documentation, with no hands-on nursing care required. Responsibilities<ul><li>Conduct comprehensive patient assessments in both in-home and virtual settings</li><li>Complete required clinical documentation and assessment tools accurately</li><li>Evaluate patient needs and determine appropriate levels of care and services</li><li>Review medical records and collaborate with care teams when needed</li><li>Maintain compliance with state guidelines and documentation standards</li></ul>Requirements<ul><li>Active Registered Nurse (RN) license in New York</li><li>Minimum 2 years of RN experience in home care, community health, or clinical assessment</li><li>Strong clinical assessment and documentation skills</li><li>Ability to manage a consistent caseload and work independently</li><li>Comfortable with hybrid work and travel for in-home assessments</li></ul>
