RN Case Manager – Travel Job

Location - San Leandro, CA
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Duration: 13 weeks

Job Role

Coordinate patient care
Discharge planning
Utilization review
Work with doctors, therapists, social workers
Create care plans and ensure proper resource use
Advocate for patients

Requirements

BLS (American Heart Association)
California RN License
Minimum 2 years RN Case Management experience
Experience in LTAC or Acute Care Hospital
Experience with MCG guidelines and discharge planning
