Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join an interdisciplinary team within a Skilled Nursing Facility. The PT will evaluate residents’ physical conditions, design individualized therapy plans, and provide rehabilitative interventions to improve mobility, restore function, and promote independence.

Key Responsibilities

• Conduct comprehensive assessments of patient mobility, functional limitations, and pain levels.
• Develop personalized treatment plans based on evaluation findings, medical history, and patient goals.
• Provide evidence-based therapy to enhance strength, balance, endurance, and overall functional ability.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and families to coordinate comprehensive care.
• Monitor progress, adjust therapy plans, and maintain accurate documentation per state and federal guidelines.
• Instruct patients and caregivers in safe mobility techniques, home exercise programs, and proper equipment use.
• Ensure compliance with all regulatory standards, including Medicare/Medicaid documentation requirements.
• Participate in care planning meetings and interdisciplinary team conferences.
• Supervise Physical Therapist Assistants (PTAs) and therapy aides when necessary.
• Maintain a safe, organized, and patient-centered work environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active state PT license (required).
• Current CPR certification (preferred or required depending on facility policy).
• Experience in SNF, geriatric rehab, or long-term care preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to work independently and effectively within a multidisciplinary care team.

Work Environment

• Skilled Nursing Facility providing long-term care and short-term rehabilitation.
• Role may involve physical activity including lifting, standing, walking, and assisting patients with transfers.
