Job Title: Hospice Social Worker
Location: Honolulu, Hawaii
Job Type: Contract, Temp-to-Perm, or Permanent
Schedule: Full-time (Days); Flexible scheduling may be available
Job Summary
The Hospice Social Worker provides compassionate psychosocial support to patients and families receiving end-of-life care. This role focuses on promoting quality of life, emotional well-being, and dignity through assessment, counseling, resource coordination, and advocacy. The Social Worker collaborates closely with the interdisciplinary hospice team to ensure patients and families receive comprehensive, holistic care.
Key Responsibilities
- Conduct initial and ongoing psychosocial assessments for hospice patients and their families.
- Provide emotional support, grief counseling, and crisis intervention throughout the hospice journey.
- Develop individualized care plans addressing emotional, social, and practical needs.
- Educate patients and families about advance directives, end-of-life options, and available community resources.
- Coordinate access to services such as financial assistance, transportation, spiritual support, and respite care.
- Act as a liaison between patients, families, healthcare providers, and external agencies.
- Participate in interdisciplinary team meetings to review care goals and patient progress.
- Document assessments, interventions, and progress notes in the EMR according to regulations and agency guidelines.
- Support families during pre-bereavement and post-bereavement phases.
- Ensure compliance with state, federal, and hospice regulatory standards.
Qualifications
- Master’s Degree in Social Work (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- Valid Hawaii state social work license or eligibility to obtain licensure.
- Minimum 1 year of experience in hospice, palliative care, home health, or related clinical setting strongly preferred.
- Strong knowledge of grief processes, family dynamics, and end-of-life care practices.
- Excellent communication, counseling, and crisis management skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Valid driver’s license and reliable transportation for home visits.
Preferred Skills
- Experience with EMRs (Hospice/Home Health EMR experience a plus).
- Familiarity with cultural practices and family structures common in Hawaii’s diverse communities.
- Bilingual or multilingual abilities are beneficial but not required.
