Job Title: Occupational Therapist (OT)

Location: San Jose, California
Job Type: Contract
Position Urgency: Quick Start
Schedule: Full-time, 36 hours/week (Monday–Friday with alternating weekend rotation)
Setting: Skilled Nursing Facility (SNF)

Job Summary

We are seeking an experienced Occupational Therapist to join a skilled nursing rehabilitation team. The ideal candidate will provide high-quality therapy services to residents requiring restorative, post-acute, and long-term care rehabilitation. This is a contract assignment offering consistent hours in a supportive clinical environment.

Key Responsibilities

  • Deliver evaluation, treatment, and documentation for patients in a Skilled Nursing Facility.
  • Develop individualized treatment plans to improve functional independence and quality of life.
  • Collaborate closely with Physical Therapists, Nursing staff, and interdisciplinary team members.
  • Maintain accurate and timely documentation within the facility’s EMR system.
  • Participate in care planning, progress reviews, and family education as needed.
  • Provide therapy services in alignment with federal, state, and facility guidelines.

Schedule & Workload

  • 36 hours per week
  • Monday–Friday primary schedule
  • Alternating weekend rotation required

Requirements

  • Active California Occupational Therapy license (must be current).
  • Minimum 1 year of experience, preferably in a Skilled Nursing Facility setting.
  • Familiarity with SNF rehabilitation programs and geriatric patient care.
  • Experience using EMRs; PointClickCare experience preferred.
  • Strong communication, teamwork, and documentation skills.
  • Ability to work independently and effectively manage a caseload.

Preferred Qualifications

  • Skilled Nursing Facility experience highly preferred.
  • Experience collaborating with multi-disciplinary rehab teams.

  • Contract length: 6 weeks
  • Holiday coverage: Not required
  • Bilingual: Not required
