Position: LPN
Unit : LTC
Shift: Mid 5x8-Hour (15:00 - 23:30)
Duration: 8weeks
Location: Rochester, NY 14612

Start date: 04/20/2026

Travel Pay Rate: (above 75 miles)
Hourly: $20/hr W2 + $780/week (non - taxable stipend)
Gross: $1,580/weekly

Job Summary:
A Licensed Practical Nurse (LPN) in Long-Term Care (LTC) provides ongoing nursing care to residents in facilities such as nursing homes and assisted living centers. They support daily health needs, monitor conditions, and ensure quality, compassionate care for elderly or chronically ill patients.

Roles & Responsibilities:

• Administer medications and treatments as prescribed
• Monitor and record residents’ vital signs and health status
• Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
• Provide wound care and basic medical treatments
• Report changes in patient condition to RNs or physicians
• Maintain accurate documentation of care and progress
• Support care planning and implementation for residents
• Ensure compliance with safety, infection control, and facility policies
• Communicate with residents and families to provide updates and support

Required Qualifications:

• Active LPN license of NY.
• BLS required.
• Minimum 1 year of experience.
• Experience with Epic
