Shift Notes::8-430 Monday-Friday, 6 points a day, which would include SOC and revisits. Berkeley County, Jefferson County, Morgan County

Need home health experience It would be 8-430 Monday-Friday, 6 points a day, which would include SOC and revisits.

On call evenings & weekends – this rotates through the staff and currently it’s been an average of about 5 nights a month, which has typically been a weekend (2 days) and 3 other week nights. The on call could turn into a few different things. Some examples are: a patient is getting discharged from the hospital at 4pm and they need a nurse to meet them at home to either show them how to manage an IV, change wound care, etc. They may just end up getting phone calls to answer questions.
