Charting System:Home care home base is a deal breaker 

Average # of patients/day: 5-8

Points per day/week: How are points broken down 30 points per week. Each type of 
visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.

Types of Cases: Start of Care Wound Care Follow up Visits etc.

Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty

Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.

Mileage reimbursed IRS Standard rate::Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.

Locals accepted: (Y/N) What is the local radius rule Yes local accepted at the same rate.
Will there be any on call If so how much Yes 2 nights per week
Must be open to on call rotations
