Job Description:
We are seeking dedicated LPNs for skilled nursing and long-term care roles, focusing on medication administration, patient monitoring, and ADL support for geriatric/rehab patients.
Key Responsibilities
- Administer medications, wound care, catheters, and tube feedings per physician orders; monitor vital signs and report changes.
- Assist with bathing, feeding, mobility, and hygiene; supervise CNAs while documenting in EHR systems.
- Provide patient/family education, collect specimens, and support discharge planning in fast-paced SNF environments.
Qualifications
- Active LPN license: BLS required, IV certification preferred.
- 1+ years’ experience in geriatrics, rehab, or behavioural health; strong teamwork skills.
