Job Summary
Start Dates: ASAP

13-week contract
40 hours per week
Shift: Days
Hours are 8a-4:30p Monday – Friday. No weekends or holidays.
Float Requirements? Will be assigned to a specific territory but if the census drops, they may be asked to float to another location. This is very unlikely but can happen
Extension Opportunities? Yes
Is RTO Accepted? What is the Max? Yes
On-Call Requirement? No call requirement
Certs: BLS
License: MA LVN License

Experience:

2 years of experience in Home Health
Oasis is not required, HCHB is required
Case load is 6 patients per day
We are asking all staff, including contracts, to work 8a-4:30p, complete documentation after each visit and transmit documentation at end of day. RNs and PTs will have a SOC each day and 4 visits, as a minimum, if time allows additional visits could be added. PTs and RNs Case Manage.
