Description

1. Job Summary
Seeking an experienced Hospital Pharmacist for a contract assignment in Glenwood Springs. This role involves rotating day/evening shifts (40 hours/week with weekend rotation) and focuses on medication dispensing, clinical pharmacy services, and ensuring safe, effective, and compliant drug therapy management during an EMR transition from Meditech to Epic.

3. Responsibilities

Interpret and verify medication orders, maintaining accurate patient medication profiles
Prepare and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
Monitor drug therapies for interactions, contraindications, and appropriate dosing
Perform pharmacokinetic dosing per hospital protocols
Review culture and sensitivity reports to ensure appropriate antibiotic therapy
Provide clinical consultation and cost-effective therapeutic recommendations to medical staff
Document clinical interventions and report adverse drug reactions
Participate in quality improvement initiatives, audits, and departmental training programs
Ensure compliance with all federal, state, and local pharmacy regulations

4. Required Experience

Minimum 3+ years of Hospital Pharmacist experience
Hands-on experience with Epic EMR (required)
Meditech experience preferred
Prior travel experience is a plus

5. Required Education

Graduate of an ACPE-accredited School of Pharmacy (BS Pharmacy or higher)

6. Required Certifications

Active Colorado Pharmacist License in good standing
BLS (American Heart Association)
ACLS (American Heart Association)
