Job Overview

Responsibilities

• The Director, Care Management is responsible for overseeing the implementation, development, and management of the Care Management and Resource Management programs. This position ensures compliance with all regulatory requirements, establishes efficient processes for patient care and discharge planning, and provides leadership to the Care Management team. The Director works collaboratively with staff, physicians, and community resources to deliver patient-centered care and achieve optimal outcomes.

   

  Scope of the Role:

• Monitoring and maintaining 13 FTE, ROI for Hospital.
• 24/7 After Hours Call ("if there was an emergency situation, and the Case Manager in charge needed advice on how to handle an issue, they would be called. Very rare occurrence. )

Requirements

• Bachelor's in Nursing
• Masters Preferred
  • Required Experience: 3-5 Yrs. Acute Care Management.
• Would like 2-5 years of Leadership Experience as a Director or Manager of Care Management
• Max Amount of RTO: No more than 1 week off
