Key Responsibilities:

• Oversee the implementation, development, and management of Care Management and Resource Management programs.
• Monitor and manage 13 FTEs and the return on investment (ROI) for the hospital.
• Ensure compliance with all applicable regulatory requirements and internal policies.
• Provide leadership, guidance, and mentorship to the Care Management team.
• Collaborate with physicians, staff, and community resources to optimize patient care and outcomes.
• Facilitate effective discharge planning and care coordination.
• Serve as on-call resource for after-hours emergencies or complex case management situations (rare occurrence).

Position Details:

• Shift: 8:00 AM – 4:00 PM (may vary per supervisor adjustment)
• On-Call Requirements: 24/7 after-hours emergency call (very rare)

Qualifications:

• Education:
  • Required: Bachelor’s Degree in Nursing
  • Preferred: Master’s Degree in Nursing or Healthcare Administration
• Experience:
  • 3–5 years in Acute Care Management
  • 2–5 years in leadership experience as a Director or Manager of Care Management
• Other Requirements:
  • Ability to manage staff, operations, and regulatory compliance effectively
  • Maximum allowable time off (RTO): 1 week
