Description

Registered Nurse (RN) – Home Health Care Manager (Day Shift)

Location: Portland, OR
Department: Home Health
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

This is a Home Health position (not hospital‑based) and requires travel to patient homes within the service area.

🩺 Key Responsibilities

Plan, organize, and direct home health services for assigned patients
Manage a patient caseload efficiently and independently
Perform in‑home nursing assessments and skilled visits
Develop, implement, and evaluate individualized plans of care
Collaborate with physicians, therapists, social workers, and other disciplines
Provide patient and family education and support
Serve as a clinical resource to patients, caregivers, and care team members
Ensure compliance with regulatory standards and agency policies
Maintain accurate and timely documentation

📋 Required Qualifications

Active RN License in the state of practice
BLS Certification (AHA, HS‑BLS, or RQI‑BLS)
