Job Description: Occupational Therapist

Job Summary

The Occupational Therapist evaluates and treats all referred patients according to physician orders and established clinical standards. This role ensures high‑quality, patient-centered care through comprehensive assessments, individualized treatment plans, and effective therapeutic interventions.

Unit: Lymphedema
Shift Schedule: Monday–Friday, 8:00 a.m. – 4:00 p.m.

Essential Duties & Responsibilities

Patient Evaluation & Treatment

  • Consistently performs initial evaluations, including administering appropriate tests and measures to identify impairments and functional limitations.
  • Establishes individualized treatment plans in collaboration with the patient and/or caregiver.
  • Selects and administers therapeutic interventions based on physician orders and established plans of care.
  • Identifies and documents functional improvement and goal attainment through measurable subjective and objective data.
  • Updates treatment plans and goals promptly, following department standards.

Documentation & Compliance

  • Completes documentation thoroughly and within required timeframes.
  • Ensures documentation reflects skilled, reasonable, and necessary therapeutic intervention.
  • For Home Health settings, consistently complies with applicable rules, regulations, and National Patient Safety Standards.

Productivity & Department Support

  • Meets or exceeds productivity expectations while maintaining high standards of care.
  • Uses non‑productive time effectively to support department needs.
  • Demonstrates responsible utilization of resources for each patient.
  • Actively participates in departmental performance improvement (PI) initiatives.

Skills & Experience

Preferred Skills & Experience

  • Minimum of two (2) years of occupational therapy experience.

Education Requirements

Required Education

One of the following:

  • Master’s degree from an accredited Occupational Therapy program, OR
  • Bachelor’s degree in Occupational Therapy from an accredited program (if grandfathered prior to 2007).

Preferred Education

  • N/A

Certifications & Licensure

Required

  • Wisconsin state license as an Occupational Therapist
    • If not currently licensed, must obtain within three (3) months of hire.
  • American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS)
    • Must obtain within thirty (30) days of hire.
