Pharmacist
HCA Florida Gulf Coast Hospital
449 West 23rd Street, Panama City, FL 32405
Schedule: 7 on 7 off
Shift: 8:00p-6:30a
Location: Panama City, Florida
General Information
Unit Specifics:
· Number of Beds: 282 Beds with a Level III NICU, PICU and Peds
· Unit staffing description: 365 days, 24 hours/day, 7 days a week, including holidays
· Types of equipment: Meditech information system, Pyxis ES cabinets throughout hospital
· Documentation system: Meditech Magic
· Scheduling requirements: 10-hour shifts, rotating weekends, 40 hrs per week
· Patient Experience requirements: N/A
Hospital Specifics-
· Certifications: BLS and ACLS would be helpful but not required
· NICU/PICU/Peds experience a plus but not required

Must have:
PharmD or Bachelor’s degree with recent acute care hospital experience, current pharmacist licensure in Florida
Advanced skills needed:
Ability to accurately check and/or double check pediatric oral and intravenous solutions
Experience in Pharmaceutical Calculations and PK dosing of Aminoglycoside and Vancomycin
Experience in TPN calculations
Ability to solve issues with Pyxis
Procedures performed on unit:
· Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.
· Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear or potentially unsafe or inappropriate
· Interprets medication orders (verbal and written), transcribes to computerized patient medication profiles accurately, follows facility practices and procedures for documentation in the patient’s medical record and trouble shoots/clarifies Centralized Order Entry (COE) orders as appropriate.
· Prepares and dispenses medications, including sterile, chemotherapy (limited), and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines.
· Based on knowledge and expertise, develops, assists, implements, and executes all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy-directed dosing
· Issues controlled substances to patient care areas and maintains records as required by law and follows corporate policies and procedures related to the dispensing and monitoring of controlled substances
· Reads, extracts, and interprets information in patient charts accurately
· Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner, shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services and reviews medication related events that have occurred to prevent recurrences
· Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel
