Description

Registered Nurse (RN) – Case Manager (Acute Care)

Location: Torrance, CA
Schedule: Monday–Friday | 8:00 AM – 4:30 PM
Weekend Requirement: Every other weekend + holidays

🩺 Key Responsibilities

Manage a daily caseload of 18–25 patients (up to 30 on weekends)
Perform comprehensive patient assessments and care coordination
Conduct discharge planning, including SNF, rehab, home health, DME, and transportation
Utilize InterQual criteria to determine medical necessity and admission status
Complete utilization review, benefits eligibility, and prior authorizations
Coordinate with interdisciplinary teams (MDs, nursing, PT, pharmacy, social services)
Arrange ambulance transport and post‑acute services independently
Participate in family meetings and treatment‑plan development
Ensure compliance with CMS, Medicare, Medi‑Cal, and HIPAA regulations
Document accurately and timely in Epic

📋 Required Qualifications

Active California RN license (pending accepted but must be active by start)
Minimum 2 years acute‑care case management/discharge planning experience
Epic EMR experience – Required
Travel experience required (1st‑time travelers may be considered)
Strong knowledge of:
- InterQual criteria
- Utilization review
- Medical necessity
- Post‑acute placement & DME coordination
